Sara Trivette PA-C, CHES is a primary care provider for Athens Neighborhood Health Center at Hilsman Health Center, a School-Based Health Center in Athens, GA. She received her undergraduate degree from Truman State University with a BS in Health Sciences and a Certified Health Education Specialist certification. In 2006, she graduated from the University of Iowa’s Masters in Physician Assistant Studies program. She was a National Health Service Corp scholar and began her career in primary care in Albany, GA with Albany Area Primary Health Care (AAPHC). She has continued to work at Federally Qualified Health Centers throughout her career.

In 2013, AAPHC opened their first two School-Based Health Centers through grant funding from PARTNERS for Equity in Child and Adolescent Health. Sara served as the primary care provider and health educator at Turner Elementary School Based Health Center from 2013-2022. During that time, she served as the department chair for School Based Health Centers and saw their footprint expand to 8 SBHCs across multiple school districts in Southwest Georgia. She worked to replicate a Hallways to Health framework across the school-based health centers; working to take healthcare outside the walls of the clinic focusing on sustainable system, policy and environmental changes to help ensure health and equity for all students. Additionally, the SBHCs focused on ensuring quality care measures were being evaluated and met.

In 2016 she was a Leadership Fellow for the School Based Health Alliance. In 2023 she completed a Healthcare Transformational Leadership Fellowship through Morehouse School of Medicine. She has worked to transform her workplace to increase Social Determinants of Health screenings and coordination of care to needed resources. Working in school-based health is her passion. She loves the opportunity to increase access to quality healthcare for all students and their families.

She is married to Brian Trivette, a high school teacher and baseball coach and has 2 young children, Ruth and Howie.
